How to Apply

Apply through a letter (on letterhead) to Municipal Director of Department of Social Welfare and Community development. 

 Requirements

 Attach to the application letter;

  1. Registrar General's
    1. Certificate of Incorporation
    2. Certificate to Commerce Business
  • Regulation
  1. Constitution of the Organization
  2. Profile of the Organization
  3. Any Brochure/Publication

Processing Procedure

  1. The Municipal Director of Department of Social Welfare and Community Development prepare Social Investigation Report and submit to District Chief Executive.
  2. The application and the report are forwarded to National Director of Social Welfare Department for certification through the Regional office.
  3. Certificate is issued after one (1) month of submission of application subject to proper and timely submission of requirements.
