Apply through a letter (on letterhead) to Municipal Director of Department of Social Welfare and Community development.
Requirements
Attach to the application letter;
- Registrar General's
- Certificate of Incorporation
- Certificate to Commerce Business
- Constitution of the Organization
- Profile of the Organization
- Any Brochure/Publication
Processing Procedure
- The Municipal Director of Department of Social Welfare and Community Development prepare Social Investigation Report and submit to District Chief Executive.
- The application and the report are forwarded to National Director of Social Welfare Department for certification through the Regional office.
- Certificate is issued after one (1) month of submission of application subject to proper and timely submission of requirements.
